8 sleep and mattress pros say these are the best mattresses for back pain

Back pain is bad enough to cope with, but when it comes to getting eight hours of restorative sleep, it can often present an extra challenge. Depending on your mattress’s design, your back might not be getting the support it needs to help alleviate pressure and get the support it needs to reduce pain and stiffness.

Luckily, there are plenty of great options for mattresses for back pain, according to mattress testers, doctors, and other sleep experts. We’ve talked to eight of them to get their picks for the best mattress for back pain and learn a little more about what really works in a mattress when it comes to getting your ZZZs.

What to look for in a mattress if you have back pain

“For those with chronic pain, I would look for good pressure relief and support from a firmer mattress,” says Dr. Daniella Marchetti, a licensed clinical psychologist and PSYPACT-certified sleep specialist who specializes in treating insomnia, CPAP adherence, and circadian rhythm disorders in adults and children.

“A soft mattress will let you sink and and curve your spine unnaturally or create pressure at your joints,” she says. “The goal should always be good spinal alignment.”

One thing to stay away from? Gel mattresses, says chiropractor and certified strength and conditioning specialist Matt Tanneberg.

“They tend to get misshapen quickly and will get indented, causing problems to your spine and non-neutral postures while you’re sleeping,” he says. “This can also cause problems when sleeping with a pattern as each side will begin to have different wear patterns.”

FAQ

Experts generally recommend a medium-firm mattress that balances support and cushioning. Medium-firm mattresses help keep your spine aligned without letting your body sink too deeply, which can make your back more uncomfortable.

Proper mattress support helps maintain neutral spinal alignment, reducing strain on your lower back and hips during sleep. The result? Relief for your pressure points and improved comfort.

You have options no matter which one you choose—you’ll want to check out each to see what’s right for you.

  • Memory foam: Conforms to body shape and relieves pressure
  • Innerspring: Firmer support with better airflow
  • Hybrid: Combines foam comfort with coil support for balanced alignment and responsiveness

A very firm mattress isn’t always better, since too firm of a mattress can create pressure points (especially for side sleepers). Research and expert guides show that medium-firm surfaces often deliver the best combination of support and comfort for back pain.

Lower back pain often comes from poor lumbar support and spinal misalignment. Test out mattresses to find one that supports the lower spine without excessive sinkage. Also, check out options with zoned support or reinforced lumbar areas.

Supportive mattresses can improve comfort and sleep quality, according to experts, which may reduce chronic back pain symptoms. While a new mattress might not cure underlying medical conditions of back pain, many sleepers experience less pain and better rest when switching to a mattress that’s purpose-designed for pressure relief.

Look for a mattress with a long sleep trial so you can test it in your preferred sleep position over several weeks or even months. Plan to sleep on it for at least 30 days so you can get an idea of how you’d sleep on it over time.
